• RADIATION: total marrow irradiation
• DRUG: melphalan
• PROCEDURE: peripheral blood stem cell transplantation
• BIOLOGICAL: filgrastim
• GENETIC: fluorescence in situ hybridization
• GENETIC: cytogenetic analysis
• DRUG: cyclophosphamide
• PROCEDURE: autologous-autologous tandem hematopoietic stem cell transplantation
• DRUG: lenalidomide
