- **DRUG:** fentanyl, propofol, atracurium
- **DRUG:** lidocaine , adrenaline, ketamine bolus, midazolam
- **DEVICE:** catheter with 3-level-injection
- **DRUG:** sevoflurane and 50% oxygen in air
