- **DRUG:** patients take 0.25 mg of digoxin daily except friday
- **DRUG:** patients take 0.25 mg of digoxin daily except thursday and friday
- **DRUG:** patients take 0.125 mg of digoxin daily
- **DRUG:** digoxin dose is calculated using Jusko-Koup method and given daily
