Every US county's chronic disease + risk behavior + prevention measures from CDC's PLACES program — 229k county-measure-year rows covering ~3,000 counties × ~30 measures × multiple years. Measures include: arthritis, asthma, binge drinking, smoking, BP-high, BP-medicated, cancer, COPD, diabetes, cholesterol screening, colon screening, dental visits, depression, kidney disease, mammography, mental health, obesity, physical inactivity, sleep deprivation, stroke, teeth removed. Each row carries crude prevalence + 95% confidence intervals, total population, 18+ adult population. Surfaces county-level health disparities — diabetes belt, mental-health deserts, screening gaps.
