Every chronic-disease prevalence, risk-behavior, prevention measure, and disability indicator at the US census-tract level — 3,047,284 rows from CDC's PLACES (Population Level Analysis and Community Estimates). Model-based small-area estimates from BRFSS + NHANES + ACS. Per row: state, county, 11-digit tract GEOID, category (Health Outcomes / Risk Behaviors / Prevention / Disability), measure (e.g. BPHIGH high blood pressure, OBESITY, MAMMOUSE mammogram use, DEPRESSION, etc), crude and age-adjusted prevalence %, 95% confidence intervals, total population. Companion to cdc_places_county (county-level we already have). Joins via county_fips with cms_hospital_service_area, fema_disasters, FQHC enrollments.
