Every entity enrolled in the federal 340B Drug Pricing Program — 93,022 covered entities (disproportionate-share hospitals, FQHCs, critical-access hospitals, Ryan White / hemophilia / STD / TB clinics) buying outpatient drugs at deep federal discounts, plus all 391,634 contract-pharmacy arrangements (45,059 distinct pharmacies) that dispense those discounted drugs — the dataset behind the 340B contract-pharmacy-explosion controversy. Per entity: 340B ID, type, participating status + start date, HRSA grant number, address, primary NPI (+ aggregate NPI/Medicaid counts), and the full contract-pharmacy list with begin/termination dates. Reverse lookup: every covered entity using a given pharmacy. Institution-level only (contact-person PII dropped). Joins CMS/NPPES by NPI.
