Per filing: OSHA record ID + establishment ID, establishment name + parent company + EIN, street address, NAICS code + industry description, annual average employees, total hours worked, total deaths, total injuries, days-away-from-work cases, restricted-duty cases, total DAFW days, illness breakdown (skin disorders / respiratory / poisonings / hearing loss / other).
