Every severe workplace injury (amputation, in-patient hospitalization, or loss of an eye) reported by employers to OSHA since 1/1/2015 — 103,745 reports through Aug 2025, including 27,386 amputations and 84,684 hospitalizations. Each names the employer, address, NAICS, body part, mechanism (event), source object, and a free-text narrative of what happened. Federal OSHA jurisdiction only. Source: osha.gov/severe-injury-reports — a single CSV in a ZIP, updated periodically. The corporate-side analog of MSHA mine accidents.
