This Penalty Complaint and Notice of Opportunity for Hearing is issued to P&M Petroleum Management for violations of its UIC permit by failing to submit the 2007 and 2008 annual monitoring reports to EPA and failing to timely submit a 2008 annual fluid analysis monitoring report to EPA for 3 wells located on the Uintah and Ouray Indian Reservation.
The wells are Federal #42-6X; East Red Wash #4-6; and Amerada Guinand Federal #1-8-25.
