The violations include operating and maintaining a Class V disposal facility which, through its injection activity, may have allowed movement of fluid containing contaminants in concentrations above the maximum contaminant level for primary drinking water standards into underground sources of drinking water; and failure to close or retrofit the Class V disposal system in a manner that would keep contaminants from entering a USDW.
