Respondent failed to dispose of all wastes composed of building materials in accordance with Part I.A.5 of the Permit. Respondent failed to inspect erosion and sedimentaion control facilities within 24 hours of a rain event of 0.5 inches or greater, as required by Part I.B.1 of the Permit. Respondent failed to operate the storm water controls at optimum efficiency, as required by Part I.C.2. of the Permit. Respondent failed to properly operate and maintain facilities and systems of treatment and control which are installed to achieve compliance with the Permit, as required by Part II.C.1 of the Permit.
