Respondent failed to include as an attachment or within the contract to lease target housing the Lead Warning Statement as required for all leases. Respondent failed to include a statement as required disclosing the presence of known lead-base paint or lead-based paint hazards. Respondent failed to include a list of records and reports pertaining to lead-based paint and/or lead-based hazards in the housing that have been provided to the lessee
