Compliance Order issued 8/8/2003. Order states: Violation 1- Respondent did not demonstrate the mechanical integrity of thirty-nine wells at least once every five years. Violation 2 - Respondent temporarily abandoned the wells for a period of more than two (2) years without plugging and abandoning the well in accordance with an EPA-approved plan or demonstrating the mechanical integrity of the wells at least once every two (2) years during the period of temporary abandonment. Violation 3 - Respondent failed to submit to EPA the required annual monitoring reports. Violation 4 - Respondent failed to submit annual reports to EPA since 1994 for the Big Sinking SWD Facility West #93.
