Failure to meet effluent limitations set forth in the National Pollutant Discharge Ellimination System (NPDES) permit, and for failure to provide proper operation and maintenance to the plant, all against its NPDES permit. The violations taken in the Complaint occurred within the period of January 1, 2001 thru March 31, 2003.
