AO issued 10/30/2003. Respondent failed to comply with Part IV.D.2.c(2) of the Permit by failing to minimize the tracking of sediments at the construction entrance and exit at its Facility. Respondent failed to comply with Parts IV.D.4 and Part IV.D.4d. of the Permit by failing to provide reports of inspection as required. Respondent failed to comply with Parts VI.D and VI.N of the Permit by failing to take all reasonable steps to minimize or prevent any sediment discharges which may have an adverse impact on human health or the environment, and by failing to properly maintain protection at the outfall of the sedimentation pond
