This facility failed to submit a completed Emergency and Hazardous Chemical Inventory Form to the State Emergency Response Commission, the Local Emergency Planning Committee, and the local fire department with jurisdiction over the facility for the 1997-1999 calendar years, by the March 1 deadline.
