This administrative order for compliance is issued to the Evanston Lodge #2588, Benevolent and Protective Order of Elks, for failure exceeding the MCL for total coliform bacteria, failure to collect a set of repeat samples following a total coliform positive routine sample, failure to provide public notice of the violations, failure to report total coliform bacteria MCL violations to EPA by the end of the next business day after discovering the violations, failure to report total coliform monitoring violations to EPA within 10 days of discovering the violation, and failure to report NPDWR violations to EPA within 48 hours.
