VIOLATIONS: Violate Osage underground injection control (UIC) program regulations by maintaining two injection wells in a manner that could allow fluid movement into underground sources of drinking water. Also, violated UIC program regulations by failing to submit required annual operations reports for 16 injection wells.
REMEDY: Complete appropriate actions on two wells to protect underground sources of drinking water within 30 days after the effective date of the final administrative order. Submit required annual operation report for the period September 2002 through October 2003 within 30 days from the effective date of the final administrative order.
