Respondent violated EPCRA 313 by failing to submit a Form R to U.S. EPA and to the State of Indiana for different chemicals in 1998, 1999 and 2000. Respondent completed and submitted all required Form Rs to the appropriate parties and paid an $11,045 penalty.
