Violations: 1. Failed to post the permit notice and a copy of the completed Notice of Intent (NOI) at the main entrance of the construction site. 2. Storm Water Pollution Prevention Plan (SWPPP) was not properly signed and certified. 3. Failed to properly select, install, and maintain control measures. 4. Failed to remove sediment at a frequency necessary to minimize off-site impacts. 5. Failed to prevent litter and debris from becoming a pollutant source. 6. Failed to describe measures to prevent discharge of dredge/fill materials to waters of the U.S. in the SWPPP.
