Violation: 1. Failed to describe all pollution control measures that will be implemented to control pollutants in storm water discharges in the Storm Water Pollution Prevention Plan (SWPPP). 2. Failed to describe interim and permanent stabilization practices in the SWPPP. 3. Failed to remove sediment at a frequency necessary to minimize off-site impacts. 4. Failed to conduct required site inspections. 5. Failed to post the permit notice and a copy of the completed Notice of Intent (NOI) at the main
