This facility conducted an audit and found that they had at least one extremely hazardous subtance above the threshold planning quantity and had not reported under Section 302 and 303 of EPCRA. Additionally they failed to submit the Emergency and Hazardous Chemical Inventory Form to the State Emergency Response Commission, the Local Emergency Planning Committee, and the local fire department with jurisdiction over the facility.
