VIOLATIONS: Violated Osage underground injection control (UIC) program regulations by failing to comply with requirements set out in EPA's approval of an extended plugging deadline for an injection well. Specifically, failed to conduct monitoring and reporting required to assure that fluids would not migrate through the well bore into underground sources of drinking water.

September 1, 2004.
