Facility was inspected on 7/8/04. NW 21st street (facility location) is adjoining a storm drain the east right-of-way of highway 95 north. From the facility a pathway runs west along 21st street intersection of Hwy, then north along curb of highway 95 approx 45 feet into the storm drain. There were several violations in the plan and physical inspection.
