VIOLATIONS: Facility violated Section 301 of the CWA, failed to sign and certify documents and failed to follow reporting instructions for DMRs and No Discharge Listings for monitoring periods 1998-1999.
RELIEF:(a) within 30 days take corrective action; (b) within 30 days submit written report on corrective actions, (c) if not possible to correct the violation within 30 days, submit comprehensive plan to do so.
