1. Facility constructed and operated deep storm drainage wells (that could impact drinking water). Facility failed to provide well inventory information and additional information when required to by EPA. Facility continued to use unauthorized storm water disposal wells. 2. Faility must provide inventory information and additional information requested. The penalty in this case is $20,000.00.
