Ivie & Associates, Inc. (NMU001423): A compliance inspection was conducted by NMED on May 13, 2004, at the site of the Madyson Square Blockbuster Video Store in Axtec, NM, to evaluate compliance with the Storm Water Construction General Permit. It was determined that, as an operator with overall operational control and day-to-day control of those activities at the project which are necessary to ensure compliance with the storm water pollution prevention plan (SWPPP), Ivie & Associates, Inc., had neither developed a SWPPP nor applied for coverage under the general permit. Because they met the criteria, an Expedited Settlement Offer was drafted to address these violations.
