This facility failed to submit the Material Safety Data Sheets to the State Emergency Response Commission (SERC), failed to submit the Emergency and Hazardous chemical Inventory form to the SERC, the Local Emergency Planning Committee, and the local fire department with jurisdfiction over the facility for the 2002 and 2003 calendar years, by the march 1 deadline. They alsdo failed to submit the Risk Managemnt Plan by June 2002.
