On August 12, 2004, Region 5 filed a Consent Agreement and Final Order with Chicago Faucet Company, 2100 South Clearwater Drive, Des Plaines, Illinois 60018. The CAFO resolves the company�s alleged violations regarding failure to, 1) make arrangements with local emergency authorities, 2) maintain adequate contingency plan, 3) provide adequate training, 4) properly label hazardous waste storage tank, and 5) file a Part A application for a hazardous waste storage permit within 30 days after its first noncompliance with any condition for an exemption from a permit in 35 IAC � 722.134. The CAFO proposes a civil penalty of $6,655.
