MOSHEN MIREHAMI (NMU001441): A storm water compliance inspection was conducted by EPA at a residential construction site on October 26, 2004, to evaluate compliance with the Storm Water Construction General Permit (CGP). It was determined that Mr. Mirehami, who is identified as an operator, had not developed a Storm Water Pollution Prevention Plan (SWP3), nor had he submitted a Notice of Intent (NOI) to apply for coverage under the CGP. Because he met the criteria, an Expedited Settlement Offer (ESO) was drafted to address these violations.
