The facility failed to implement their Risk Management Program. Some of the more serious violations include: Not conducting a compliance audit, not addressing problems identified in the hazard review and not preparing written operating procedures as specified by the regulation. Thus EPA issued an Expedited Settlement Agreement on 12/21/2004. The penalty being sought is $1275.00
