The facility failed to implement their RMP Program. Some of the more serious violations include not responding to all deficiencies stated on the audit, not having procedures that address consequences of deviation and steps required to correct or avoid deviations, and not establishing a system to identify and resolve problems indicated during the process hazard analysis. A penalty of $900.00 is being sought.
