Queen Louise Home failed to comply with the requirements of a previously issued AO, ordering Respondent to submit radionuclide contaminants monitoring results (or historical data) within 45 days of the receipt of the AO. Only the first quarter of radionuclide contaminants monitoring results was submitted. A Quick Settlement Offer has been included, if Respondent completes the remaining 3 quarters of radionuclide contaminants monitoring by June 30, 2009 and pays a penalty of $200.00.
