NPDES Permit ID0024422 ( Permit ) was issued to the Respondent on October 28, 1999. The Permit specified the conditions under which the Respondent may discharge treated effluent from the facility. The effective dates of the Permit were November 30, 1999, to November 30, 2004. Respondent failed to apply for a new permit in a timely manner as required under 40 C.F.R. Part 122.21(d)(1). Due to Respondent�s failure to re-submit an application in a timely manner, the Permit was neither administratively extended nor re-issued.
