Respondent is an ice manufacturing facility that had an NPDES permit for discharges, but failed to apply for the permit to be reissued prior to expiration of the permit on June 30, 2004. This resulted in the permit expiring and Respondent discharging without a permit. Additionally, prior to permit expiration, Respondent failed to properly take and report samples for Total Residual Chlorine and pH on Discharge Monitoring Reports. Respondent failed to have a properly authorized person sign and certify Discharge Monitoring Reports. Respondent failed to properly date Discharge Monitoring Reports.
