The facility voluntarily disclosed to U.S. EPA that it distributed a chemical subject to a Low Volume Exemption on three occasions to a customer without providing written notice to the customer of the use and processing restrictions on the chemical. The facility also voluntarily disclosed to U.S. EPA that it exported chemicals on 19 occasions without filing the required Export Notices with U.S. EPA.
