This is an Administrative Order on Consent pursuant to the Clean Air Act issued to Surprise Valley Health Care District. The violation involves failure to conduct and submit to EPA initial performance test data as required by section 62.14463(a); failure to submit to EPA values for the site-specific operating parameters for the operating incinerator as required by section 62.14463(b); failure to submit a waste management plan as required by section 62.14432; and failure to submit annual reports as required by section 62.14464(b). The Order directs Surprise Valley Health Care District to permanently shut down their hospital/medical/infectious waste incinerator. They are to achieve this shut down by following the steps and meeting the deadlines outlined in a compliance plan which is included as Attachment A to the AOC and incorporated by reference.
