This proposed order and penalty complaint with notice of opportunity for hearing is issued to LT Logging for failure to submit a written schedule for retrofitting the facility and/or plugging the drains; and for failure to close or retrofit the Class V disposal system in a manner that would keep contaminants from entering a USDW.
