Respondent failed to include a Lead Warning Statement, a statement by the lessor disclosing the presence of lead-based paint and/or lead-based paint hazards, a list of any records or reports available to the lessor that pertain to lead hazards, a statement by each lessee affirming receipt of the information required, and the signatures of the lessor and the lessee certifying to the accuracy of their statements.
