This administrative order is issued to Brooks Lake Lodge for failure to monitor for total coliform bacteria; failure to collect a set of repeat total coliform samples after a routine total coliform positive sample; exceeding the total coliform bacteria maximum contaminant level; and failure to report violations to EPA.
