Based upon a 9/9/2005 inspection, EPA believes that the AWTP is in violtion of the following risk management program elements: management, hazardous assessment (ie., OCA parameters, alternative release, population, documentation). Prevention Program (ie., process hazard analysis, operatiang procedures, training, mechanical integrity, management of change,compliance audit), Hot Work Permit, Contractors and Emergency Response.
