EPA inspected Chemtrade's facility in Kalama, WA, on 7/21/05 and found many program deficiencies including: missing documentation regarding their offsite consequence analysis, inadequate operating procedure review, inoperable managment of change program, inadequate follup on their compliance audit and underreporting their RMP chemical quantity.
