Three EPA representatives inspected the City of Astoria Wastewater Treatment Facility on July 11 and 15th, 2005. Based upon this inspection, EPA believes City of Astoria Wastewater Treatment is in violation of the following risk management program elments: Hazard Assessment, Process Hazard Analysis, Operating Procedures, Training, Management of Change, Pre-startup Review, Incident Investigation, Compliance Audit, Employee Participation, Contractors and Updates.
