Establishment has submitted late annual pesticide production reports twice in the last three years. A Notice of Warning was issued in 2004 which notified the establishment that a civil penalty would be issued if their annual report was late in the future.
This year's report, due to the EPA by 3/1/2005, was received on 3/7/2005.
