NPDES Permit ID0021831 ( Permit ) was issued to the Respondent on 09/30/1999. The Permit specified the conditions under which the Respondent may discharge treated effluent from the facility. The effective dates of the Permit were 11/02/1999 to 11/02/2004. Respondent failed to apply for a new permit as required under 40 C.F.R. Part 122.21(d)(1). Due to Respondent's failure to re-submit an application the Permit was neither administratively extended nor re-issued.
