## Summary

Administrative Order(AO)for failure to collect a Cryptosporidium matrix spike sample for its June through September 2008 Cryptosporidium field samples which invalidates these samples.  The AO requires the system to sample collect a  Cryptosporidium matrix spike sample and Cryptosporidium, E. coli and turbidity field samples in June though September 2009,and submit a bin classification to Region 5 U.S. EPA.  System met the requirements of the AO by submitting a revised source water monitoring plan and sampling schedule and collected Cryptosporidium, E. coli and turbidity samples during June, July, August and September 2009, and a Cryptosporidium matrix spike sample on the same day as the June Cryptosporidium field.  System reported the sample results and system’s initial bin classifications to U.S. EPA.  System also provided public notice for the violations cited in the AO.  EPA issued a letter terminating the AO on 11/9/2009.
