The McCain Foods USA facility located in Othello, WA self-disclosed on April 30, 2009 for failure to file Form Rs for chromium for reporting years 2006 and 2007. The facility submitted documentation addressing the nine criteria of the Audit Policy which qualified them for a waiver of gravity-based penalties for the EPCRA 313 violations.
