The Heath Tecna, Inc. facility located in Bellingham, WA self-disclosed on July 8, 2009 for failure to file Form R/As for tetrabromobisphenol-A for reporting years 2003 through 2007. The facility submitted documentation addressing the nine criteria of the Audit Policy which qualified them for a waiver of gravity-based penalties for the EPCRA 313 violations.
