In the Complaint Region 5 alleges violations regarding failure to, 1) to use a properly completed uniform hazardous waste manifest (EPA Form 8700-22) when shipping hazardous wastes and, 2) to apply for and obtain a hazardous waste storage permit, when Creative Liquid Coatings failed to comply with permit exemption conditions of 90-day storage, container labeling and dating as specified in 329 IAC 3.1-7-1, 40 CFR § 262.34(a) for a period from 03/02/2005 to 06/22/2005.
