In the Complaint Region 5 alleges violations regarding failure to, 1) to use a properly completed uniform hazardous waste manifest (EPA Form 8700-22) when shipping hazardous wastes and, 2) to apply for and obtain a hazardous waste storage permit, when Elite Enterprises, Inc. failed to comply with permit exemption conditions of 90-day storage, container dating, employee training, contingency plan, and weekly inspections as specified in 329 IAC 3.1-7-1, 40 CFR § 262.34(a).
